INDICATION: disease staging191.1 Bifrontal or frontal glioblastoma multiforme
COMPARISON: Multiple priors, most recent from 10/4/2013
Multiplanar, multi-sequence MR imaging of the entire brain was performed before and after intravenous administration of gadolinium-based contrast.
-Calvarium/skull base: Status post left frontal craniotomy. No focal marrow replacing lesion suggestive of neoplasm.
-Orbits: Grossly unremarkable.
-Paranasal sinuses: Imaged portions clear.
-Brain: The anterior left frontal gyri abutting the resection cavity demonstrate slightly increased thickening, FLAIR signal, restricted diffusion and enhancement. Perfusion imaging in this region is limited secondary to susceptibility artifact. No significant white matter disease or acute ischemia. No mass effect, hemorrhage, or hydrocephalus. Grossly normal flow-related signal in the major intracranial arteries and dural sinuses.

Post surgical changes related to left frontal craniotomy for resection of a glioblastoma. The anterior left frontal gyri abutting the resection cavity demonstrate slightly increased thickening, FLAIR signal, restricted diffusion and enhancement. These findings raise concern for disease progression, although treatment related changes can have similar appearance.
